The Florida Department of Health in Bay County in collaboration with the Bay County Health Equity Taskforce and Community Partners is conducting a PACE-EH Survey for the Glenwood Community. This survey will help in prioritizing community needs and projects. They are asking for your help in getting the word out by sharing the below surveys.
